- PSA/NSO-certified birth certificate (photocopy)
- Recent 2×2 ID photos (4 copies)
- Medical clearance or health certificate
- Good moral certificate
- Photocopy of parent/guardian valid ID
For New Students (Freshmen & Transferees)
- Report Card (Form 138)
- Certificate of Transfer / Honorable Dismissal
- Placement test results (if applicable)
- SEN Needs Assessment (for students with accommodations)
For Returning Students
- Previous school year’s Form 138
- Updated Student Information Sheet
- Finance Office clearance
