General Requirements

  1. Duly accomplished enrollment form (available onsite or via download)
  2. PSA/NSO-certified birth certificate (photocopy)
  3. Recent 2×2 ID photos (4 copies)
  4. Medical clearance or health certificate
  5. Good moral certificate
  6. Photocopy of parent/guardian valid ID

For New Students (Freshmen & Transferees)

  1. Report Card (Form 138)
  2. Certificate of Transfer / Honorable Dismissal
  3. Placement test results (if applicable)
  4. SEN Needs Assessment (for students with accommodations)

For Returning Students

  1. Previous school year’s Form 138
  2. Updated Student Information Sheet
  3. Finance Office clearance

Additional Requirements for Scholarship Applicants

  1. Scholarship application form
  2. Income Tax Return (ITR) or Certificate of Indigency
  3. Recommendation letter (community leader or principal)
